Forms

A form in Sign Ups is a link to something families fill out somewhere else: a CCB form, a DocuSeal waiver or any web page. Sign Ups doesn't build forms. It connects to the ones you already have and tracks who's done them.

Go to Sign Ups > Settings > Forms. You need to be an Administrator, or a Ministry Leader of the event's ministry.

The three types

TypeUse it forWhat sync does
RegistrationThe CCB form families register onCreates registrations, people and any payment
PaymentA CCB form only for paying a balanceAdds payments to people already registered
ReleaseWaivers, medical releases, photo permissionNothing. You tick it off, or DocuSeal does

Only choose Registration for real sign-up forms. A payment-only form set as Registration can create registrations you didn't expect.

Add a CCB registration form

Before you start, make sure the CCB form has at least one submitted response. Submit a test response yourself if it's brand new. Sign Ups reads the questions from existing responses.

  1. Pick the event in the event picker.
  2. Go to Settings > Forms and click Create.
  3. Pick Form Type: Registration.
  4. Type a Form Name.
  5. Paste the CCB form's link into External Link. You should see "CCB Form ID detected".
  6. Click Create Form. It's attached to the selected event.
  7. Map the questions (next section).
  8. Click Sync.

Map CCB questions

Mapping tells Sign Ups which CCB question is the grade, a parent's email, allergies and so on.

  1. Open the form and find CCB Field Mapping.
  2. For each question, pick where the answer goes. Sign Ups guesses the obvious ones for you.
  3. For anything you want to see that isn't in the list, pick + Create custom field.
  4. Pick Ignore for questions you don't need.
  5. Save.

Names come from CCB automatically, so you don't map them.

If you add questions to the CCB form later, click the refresh icon to load them.

Mapping only applies to new registrations. To fill in a newly mapped question for people already imported, use Resync from CCB.

Ask about the grade the right way

Ministry Toolkit stores the grade a student just finished. Ask "What grade did your child just complete?" on your form. A question like "Grade this fall" puts everyone a year ahead. See Grades.

One CCB form for several events

If one CCB form covers several events (like a summer form with a ticket for each camp):

  1. Open the form and add each event under Assigned Events.
  2. Under Response Filter, set which answers or tickets belong to each event.
  3. Save.

Add a release form

  1. Click Create, pick Release, name it and paste any link (Google Form, PDF, CCB form or DocuSeal).
  2. Click Create Form.

Each release form gets its own checkbox column on Registrants. Tick it off as forms come in, or connect DocuSeal to tick them for you. Families see outstanding release forms with Click to Complete on their Family Portal.

Add a payment form

Create it as Payment with the CCB payment form's link. Payments on it are matched to people already registered. A payment for someone not registered yet is skipped and tried again on later syncs.

Adding a Payment form with a link also turns on Make a Payment on the Family Portal.

DocuSeal waivers

With DocuSeal connected:

  1. Create a Release form with the DocuSeal link.
  2. Open it and set up DocuSeal Integration: pick the Template and the Registrant Name Field (the question that has the child's name).
  3. Optionally map medical answers under Import to Profile.

Each sync marks the form done for registrants whose parent signed. Medical answers only fill in empty fields.

Remove a form from an event

Open the form and remove the event under Assigned Events. Forms can't be deleted, so they're available to attach again later.

Common questions

My form isn't in the list. The list only shows forms attached to the selected event. Switch events or use the type pills at the top.

"Submit at least one response to this CCB form to configure field mapping." The form has no responses yet. Submit a test response in CCB, then click the refresh icon.

Registrations aren't importing. Check the form type is Registration, the event is Open and the CCB API user has form_responses turned on.
